Documents can be submitted using the EPIC-LA system at the following URL: https://epicla.lacounty.gov/SelfService/#/home. The documents must be submitted to the EPIC-LA plan case number shown at the upper left corner of your GMED review sheet. Please contact GMED staff at (626) 458-4925 if documents cannot be uploaded to the plan case.
