The actual forms are available free of charge at the LA County Department of Public Works - Survey Division Public Counter on the 4th Floor, 900 S. Fremont, Alhambra, CA. We do not have a .pdf on our web site because the forms are on heavier card stock (50#) as opposed to 20# bond paper. Also, the forms are two sided. Often, when the form is downloaded from a .pdf, we will get two single pages on bond paper which is unacceptable.
